DESIGN: Emerald Trace embraced a grand undertaking by deciding to change its internal culture as well as physical environment from institutional care facility to a household model. They knew this change would give their residents more options, less stress and therefore heighten their quality of life.

Emerald Trace was the second phase in their change to a household model. The new campus consists of two two-story buildings both with a walkout level.  The first building is dedicated to two short term stay rehabilitation households, with administrative offices and rehabilitation suites in the walk out level.  The second building has one long term skilled care household and one memory care household with the central kitchen and maintenance areas in the walkout level.

Phase three consist of additional assisted living, skilled care and independent living apartments.
